How to Generate Paper-Based Source Forms for a Visit
1. Start the form generation workflow 0:22
Go to Forms.
Choose the visit type you want to generate forms for, such as Baseline or Screening.
Select the appropriate language and trial.
Click Quick Create to begin generation.
2. Let Clinials AI identify the required visit content 0:33
Clinials AI reviews the protocol automatically.
It analyzes the Schedule of Activities and study procedures.
The system identifies the required:
Activities
Assessments
Forms
Worksheets
These are generated specifically for the selected visit.
3. Review the generated visit in the Content Hub 0:52
After generation is complete, open the Content Hub.
Find the generated visit there.
Note that Clinials may use the protocol’s terminology for the visit name.
Example: a Randomization visit may appear instead of Baseline if the protocol does not define a baseline visit.
4. Preview the forms before exporting 1:03
Open the visit to preview all generated materials.
Review the forms and worksheets for that time point.
Confirm the content before downloading or exporting.
5. Download or print the final source forms 0:14
Export the completed set as a DocX file for editing.
Or export as a PDF for printing.
Use the final forms during patient/site visits.
The output includes paper-based source forms such as:
Eligibility worksheets
Vital signs forms
Adverse event forms
Visit procedure forms
Other protocol-required assessments
